I got the forms themselves from another website, but all the dorky stuff written in the blanks is my own.
I really like the Luke Skywalker and Snape ones, but they’re all funny.

Cersei and Jaime are from George R.R. Martin’s “A Song of Ice and Fire” series. And yes, he’s her little brother but it’s not quite as weird as it sounds – they’re not kids. (Well, it is still pretty weird… xD)
